I agree that the above information is accurate and is to be used as the basis for assessing liability agnst the above named person in the event that my dog is found to be vicious.

Dogs are not allowed on the premises.

The dog shall remn in a securely fenced area and shall not leave the enclosure during the period of the trial without the express permission of the judge or court administrator.

No person shall approach the enclosure with food or water.

No person shall approach the enclosure in order to feed or water the dog.

No person shall bring the dog to the courtroom except through a judge or court administrator who has been informed that the dog will be present.

No person shall make statements to the judge or court administrator regarding the behavior of the dog, the judge, or the courtroom.

The person(s) having custody of the dog shall be responsible for the care and safety of the dog during the trial.

No person shall use drugs, alcohol or other intoxicants in the presence of the dog, nor shall the dog be exposed to these substances.

The person(s) having custody of the dog shall be responsible for the health of the dog and shall obtn and mntn current health records.

The person(s) having custody of the dog shall be responsible for the care and safety of the dog while in the custody of a bliff, officer or person employed by the court or court administrator.

No person shall harass, interfere with or assault the dog or the bliff, officer or person employed by the court or court administrator.

All persons in attendance at the trial must sign the enclosed affidavit to be provided to the court.

All dogs are subject to a $10 fine for flure to abide by any of the provisions of this notice.

In accordance with the statute, the owner, in a civil case, or the guardian of an incompetent owner may not be liable for any loss or damage to the property of the defendant unless caused by the dog's vicious character or the owner's gross negligence or willful misconduct, as defined by the statute.

The following are examples of a dog's vicious character:

The dog has been trned or bred to be a fighting dog,

The dog is of a breed that is known to be or is alleged to be a fighting dog,

The dog has a reputation for fighting or biting, or

The dog is known to have previously bitten another person, animal or the judge.

A court has found that a dog is "vicious" based on one or more of the above definitions.

In accordance with the statute, the owner, in a civil case, or the guardian of an incompetent owner may not be liable for any loss or damage to the property of the defendant unless caused by the dog's vicious character or the owner's gross negligence or willful misconduct, as defined by the statute.
